The rare case is reported of an isolated pulmonary metastasis presenting 5 years after the curative resection of a pancreatic adenocarcinoma. This case emphasizes the value of the gastrointestinal tumour biomarker CA 19-9 as a sensitive parameter in the detection of pulmonary metastases of pancreatic cancer, and the role of totally thoracoscopic metastasectomy in their management.
